I've been having a similar issue with my MacBook Pro Late 2013 from before Mojave, but with Mojave it went from annoying to can't live with it 50% of the time it was as if it was shutting down when it entered sleep mode.

I tried all of the different ways to get it to enter sleep, unplugging power supplies, anything externally connected - USB, displays, updating and then removing third party software, SMC, NVRAM resets you name it.

None of that worked and there was no hint as to what was causing it in console logs etc and it was still about 50-50 that sleep properly.
In the end, and out of desperation, I disabled the running of a helper program that I have been using with _no_ problems since 2011 to manage my windows more effectively. The program makes use of the macOS Accessibility framework to control the windows and there is no way it should cause this type of problem. However, I've not run it for the last week and the problem seem to have stopped.
I'm not going to mention the name of the program, as they are a tiny outfit it would be unfair to tarnish their good name when the problem is as likely to be an Apple problem as anything else. But, if you've not got a fix, then it might be worth stopping programs that make use of that framework to see if things improve. You can see these programs by looking under the System Preferences->Accessibility - Privacy tab
